On DetectiveConanWorld, it says that Shinichi said "Ran-neechan" once. I'm curious, did this really happen? If yes, which manga chapter/episode?

Yeah, that happened during the Desperate Revival arc. (Episode 192, I think) Basically, Shinichi had gotten used to being treated as a child and slipped up on honorifics.

It's why I had to edit a line in episode 5 about using -nee-chan - no one would be expected to know that the exact wording of a random line in a random episode would prove to have plot significance over 180 eps later...
