I also like the mother theory,more than the brother.But the problem is that I don't know what a shrunken mother would add to the story.
And Shuukichi Haneda would be great as Sera's brother,he's funny^^

Just a foil hat suggestion: the mystery girl being Shiho's mother. It could fit the frame why she is older than elementary school age, and it might only because it's past 11 PM here, but from the top of my head I recall being said they died in accident as the organization calls it, but had never seen the bodies, could also possibly answer why Sera was looking for Haibara

That was my immediate thought as well.
She looks a lot like Ai, seems to be quite clever, but is pretty young hence she is most obviously under the effect of the APTX.
Furthermore the way she's drawn is similiar to other foreigner in DC.
I has to be either Ai's mum or an entirely new character.
There's now character so far that would match her.

I think she looks like Ai a bit though having parts of Akai and Sera as well.
However if she's a new character again that would suck imo.
There are to many new characters at the moment with Sera's second brother/Haneda.
Sera's first appearance isn't that far away either.
I hope it's someone we didn't expect but was already mentioned by name.
Thus it could be Elena.
At least I hope so.

If it's Elena then it would mean Haibara's family is linked to Akai's...
So Akai would have dated someone related to his family -> Akemi.
But I have to admit she resembles a lot to Haibara, the mysterious "girl".

Unless Ai isn't really Akemi sister,which I doubt...
No really I can only say her behaviour looks like Haibara's,but physically she's just a mini Shuichi Akai with blond hair...this is already disturbing enough to have an other APTX victim,so I don't want to imagine a more complicated situation.
I'm just waiting to see her meeting with Conan,and if it's a shrunken adult,she'll have to meet Ai too.Maybe the confrontation between Ai and Sera planned by Gosho will be centered around the mystery girl...

But then I have to admit that if that "girl" is Haibara's mother then it would make sense since she was working on APTX4860 she could have taken the drug just like her daughter when she was in a dire situation, maybe she was about to die after the car accident and then took the drug and was rescued?

And didn't Amuro say Haibara looked a lot like her mother? The girl resembles a lot to Haibara. Plus I think all the times we saw a picture of Elena, she was wearing sunglasses, no? So we can't say what type of eyes she had, thus making us think that the mystery girl could be Elena. But currently the storyline is about the Akai family so all of this is not likely I think.

My first reaction was, "Jesus Christ, she's Haibara with Sera-esque eyes." Amuro said Haibara/Shiho looks a lot like her mother, so I can see why people think she's Elena. The problem is that she is "a long way from trusting" Conan, which wouldn't make sense considering he saved Haibara's life at least a couple of times. If I were a parent and someone saved my daughter's life a couple of times, the least I would do is trust that man. But then again, how would she know what Conan has done.
