Well, Kaito already knows Ran so there would be a problem for him to mistake the one for the other.
And they have a bit different... attitude.

The Ran=Aoko mistake I doubt would happen often b/c unlike Shinichi=KID, the difference is more than hair(hint: Aoko and Sera both lack this).
