Probably would like to know how Takeshi and the daughter of the Hatamoto family are doing after only being mentioned all the way back in volume 22.  Doubt that it'll ever happen though, since they were really just one-off characters.  I'd hate to say it, but Fusae might have the same fate. 

But good God, it's been forever since we've really seen much of Makoto, or the Kudos at the very least, by which I mean for more than a page or two.  I was thinking Gosho could switch it up a bit and bring in another Sonoko case or something, since we haven't had one in quite some time (around that red handkerchief case in volume 52, right?).

Aside from the BO members who have been glossed over (everyone dealing with Ai [her parents, particularly, imo]) or those whose backgrounds have yet to be explained, Eisuke.  I really liked his character.

I also want to see Sonoko again soon. Maybe in a bit more important case. Makoto would fit in that case nicely, too.

Also, another case with Ran (+Sonoko?) and the Detective Boys together. Possibly without Kogoro or anyone from the police to steal the spotlight.

I'd definitely like to see Okita (Heiji's kendo archnemesis) back and not as a background mention, but as an actual character? I admit I'm a Shinsengumi junkie, but I'd really like to get to know him better. C'mon, Gosho, make him show up at Heiji's doorstep with a creepy case about a murdered monk in Kyoto or some anonymous threats he has been getting or whatever and grudgingly ask for help, so we can have a nice Kyoto adventure with pretty places, maybe-possibly-kendo and a lot of Heiji. I know we have just HAD a lot of him, so maybe not know, but someday, in the future? Pretty please?

Besides Okita, BO, FBI and Sera are always welcomed by me.

And a MK special with Toichi flashback would be awesome.
