I am pretty sure the plate doesn't appear in the manga (at least not in that manga dealing with Pisco) so I wouldn't place it as part of the Detective Conan canon

Well I saw A 48 69 before but after I noticed it, I tried to see the number in different episodes but it became 43 68. I don't know if it changed or the Video must've blurred the number a little.

Still...who would name a drug that kills/grants immortality "Sherlock"  ?  Personally I'll name it Pandora xD.

I wouldn't be surprised if they gave a different meaning for 4869.

Still it's extremely intriguing, let's hope it connects this way.

What if this drug were called Sherlock because it was designed to react differently in people with superior cognitive ability?  A possible application is it could have filtered candidates for positions in the BO.  Of course those rejected would die, but I can't imagine that would bother any of those affiliated with the BO.  Also, Ai is a genius, so it would explain why she and Conan both activated the less frequent effect. 

This theory could go another way.  A plot for genocide of the weak.  Darwinism fast-forward.  APTX 4869 in the water supply.

Also, if they could target the brilliant differently than everyone else, the BO could destroy only the parts of the police force which had a chance at stopping them.

4869/3 is 1623 or ichiro-niisan.  could be intended as a cure for someone named Ichiro (although I believe Ichiro is a boys name and niisan signifies female?) and I can't explain the 3 except that 4869 is 3000 more than a potentially significant year (Rasputin's birth).
the 48th emperor of japan died in 770.  770 is an american area code in Georgia.  Georgia was named after King George II.  King George VI was born on 14 December.(now that was a stretch)
the 69th emperor of japan was born on 14 of December (1412 ~ Kaito) in the year 1009

I'd buy that.  That's a decent enough way to pick members, the original BO members must run across enough criminals, and probably fairly often they would want to deal with them (either removing other crime organizations/businesses or some criminal that tried to steal their money)

Now a more and fun speculative rant! :: What if its more of a test, to see how you would adapt in a totally messed up situation ( a test of your physical and mental capabilities )

they would want the best and smartest, and would need people with "flexible" morals so why not recruit those you would run into that are already in crime and see if they are up to the test with the 4869 initiation test :
