Maybe File 690 is like the beginning of V41F4. In V41F4, there is a suspicious car going after Yukiko's car, then Ai-chan suspects it is BO member, Akai Shuichi appears instantly from V41F6, he has a picture of Detective Youth Club and said Ai-chan looks alike someone, Akai seems like BO member who is going after Ai-chan.
On V41F7 onwards, Akai still stalking someone and Jodie and Vermouth appears almost every chapters from V41F6 until V42F10.
So in File 690, if the one hiding under the desk is Okiya, maybe it is the beginning of Okiya's suspicion just like Akai, Jodie and Vermouth back to Volume 41 to the end of Volume 42 which leads to the war between FBI and BO.

What spoilers? Nothing that isn't scanlated is mentioned outside of 690, and that's in the title so you should be able to gather that there's information pertaining to it inside.

Okiya shows up in 691. Conan hears him in the background when he calls Agasa. Okiya is over at Agasa's cooking something that looks like stir-fry. (He's got an apron on too!)

I personally think Okiya is a red herring.. Probably just a Makoto Kyugoku type of character.. Well Ai's BO sensor ticked off because he might be an ex-member.. The reason he can't be Bourbon is that it's just too obvious.. He can be somone related to Bourbon.. But he is not a bad guy..
