Worst would be the BO is completely destroyed, Conan restores himself and lives happily ever after with Ran. Also displeasingly would be Kogoro dying, something I have become increasingly (though perhaps baselessly) worried about as the show has gone on.

That one is the Best Ending ever except for the Kogoro part

An ending that leaves Conan as Conan and makes him a recurring villain with a tragic past in Magic Kaito would make me not want to follow a new Magic Kaito series.
Absolutely the worst possible ending is Aoyama stops writing abruptly and leaves it hanging. Also, any plot where Kogoro's father is half Japanese half American and is father's name is Arthur. Arthur would probably be the head of the B.O.

Kogoro dies from being poisoned in his neck so many times... xD Then Ran finds out of Conan being Shinichi and decides to kill him for murdering her father. As a result, Ran becomes the boss of the BO and is having an affair with Gin. Then, we'll find out that Agasa is a 6 years old kid who took a poison which made him grew, and after he returns to normal he becomes adopted by Jodie.
Can you think of a worst but lolest ending? xD
No, seriously, a really nice ending would suck big time. Someone must die, and better an important character. We should leave this to Aoyama-sensei, he always knows how to make us die from expectation.

Ran and Conan get caught by BO, they kill Ran and let Conan live, and Conan didn't ever get the chance to tell Ran that he was Shinichi and he loves her...
or that Ran will find it out and cannot forgive Shinishi lying over all the time.
How about Ran finally goes out with Gin as a thanks for shrinking that "Mystery Otaku"
Conan gets stuck in his body but Haibara returns to original form as she has transformed only once and Conan about 3 times.
Ayumi after knowing Conan=Shinchi Trashes him and decides to not to fall in love with anyone
and Kogoro-Eri Kill each other.

Probably the thing that would ultimately suck for an ending would be that Shinichi wouldn't return to normal. For DC to end like that after such a long time would be awful. I mean, that's one of the main goals of the series, and to have that unresolved would be infuriating.
Character deaths here and there are gonna be predictable. I like all of the characters, so deaths will be bittersweet, but I think I can deal with them...except if it's gonna be Haibara or Ran (which I could see happening).
Though I'm not for a totally happy end either, which would include everyone getting married or something like that. A mutual confession between Ran and Shinichi may likely be included...either that or Gosho will pull a Takahashi and just imply that they know it in their hearts.
Oh the worst ending would probably have to be....
Ran gets kidnapped by the BO and it is up to Conan and Heiji to find her before they kill her. Conan fails at finding Ran in time and Ran's body is missing. Conan and the FBI decide to infiltrate the BO only to be caught off guard. Heiji turns out to be the boss and Kazuha was Vermouth in disguise! 'Heiji' shoots Conan in the arm and somehow escapes. Haibara disappears in that night to never be seen again. Jodie ends up in a coma and dies a year later. To cover up the evidence, the BO burns down the warehouse where they were, burning Ran's body with it. After the BO infiltration, Conan finds out that The real Heiji and Kazuha are just regular high school students. Conan becomes depressed knowing that his friendship with Hattori was all a lie. Soon after, Kogoro 'commits suicide'. Conan knows it was the BO. After ten years, Conan commits suicide not being able to live with himself anymore.
Now it that isn't a bad ending. I don't know what is... O_O

Just thought of a worse ending, BO is actually benevolent organization who have to use extreme measures because the CIA are planning to put Japan under water because it is too much of a competition in too many industries for the US's interest. Head of BO is a misguided lovable character who discovered the plot due to his contacts at the Kremlin (I just add that because in Case Closed the dub voice for Agasa has a Russian accent).
the worst ending is simply that conan/shinichi doesn't end up with ran...
on another note... doesn't anyone find it suspicious that Hattori gave conan, a young little kid, some really strong alcohol? He introduced conan to the temporary antitdote... (so he or someone close to him is in the BO)
so the second condition of that worst ending is that hattori is the head of the BO
