You are going to disagree with me. But by remove all ladies are attached and checking Face shape. After all as one grow up the basic appearance remain somewhat the same. But to me it appears in your list Chekhov, that all the remaining ladies seem to have the wrong face shape. It might be a new character.

Hair color right ( Black ), eye shape looks right, not attached to anyone, and the face shape is possible ( Heart )

Looks like Shiratori is trying it again. That guy is taking no risks now that he found the girl of his destiny. 

And once more it was shown that Conan lowers his defense significantly around the Detective Boys. He must really feel secure having them around to talk so carelessly about the past. I wonder if they will have enough clues to figure out Conans real identity at some point (shortly before the end of the Manga. If it ever ends.)
And it looks like the Detective Boys are playing cupid once more. First Takagi and Satô, then Agasa and his first love, then Shiratori and Kobayashi-sensei and now Chiba. They must be getting really good in it. 

Also, considering all the bad talk about that "Skeleton Teacher" I wouldn't be surprised if he had discovered the love letter and kept it. At least I think the Detective Boys will have to talk to him (and find out that he is a nice person deep down somehow) to be able to solve this case. Well, that is at least my prediction.

Most of the known known female recurring characters in the ballpark of Chiba's age who might fit the bill are: Kobayashi, Sato, Yumi, Azusa Enomoto, Yui Uehara, Yoko Okino, and heck I'll even throw Phantom lady/Kaito's Mom on the list. I'm leaving Eri, Yukiko, Akemi, and Hidemi Hondou off since they look a bit different as kids. Finally there are a few non-recurring chars to consider. Chiba is a bit of a kid's show lover, so I'm going to add Tomomi Sakaguchi from the Tragedy of Gomera case (128, V13.8-10). She shows feelings for murder case culprit Shugo Matsui

We also have Natori Mishio who played Kamen Yaiba's first love interest from the Onsen muder case (722, V69.4-6). She's tied up with her manager Tanzawa who was the murder case culprit.

I may have forgot someone so I'll keep reviewing. Of course an unseen third party is possible. Maybe Chiba will be forced into a jailhouse romance?

Yumi, Yui, and Yoko have a symmetrical hairstyle. Kaito's mom is a question mark, although in the anime she has symmetrical hair. Sato and Kobayashi have more closely matching hairstyles to the girl. Azuha has long bangs and so is a possibility because cutting them might reproduce the appearance of the girl.
Yumi may have restyled her hair, but Kobayashi pre-haircut or Sato are candidates which would make for an interesting love triangle. The other two non recurring char candidates are both attached to someone who committed a murder thus making it more interesting.  We'll see how things go with the translation.

I think we can deduce a lot by the age. Japanese elementary school has 6 years, correct? So Chiba - and that girl - will have been around 12-13 years in their last year. Since the letter was written 13 years ago that means they are both about 25-26 years old now. This allows us to eliminate all the mothers with teenage children from the list already (Eri, Yukiko, Phantom Lady).

Natori Mishio is 28, that is 2-3 years too old. There is a slight chance it might be her, but unlikely. Chibas first love should be 24-26 years old at the time of her appearance.
Shugo Matsui fits that age. It might be her, but considering she already has a love interest it would go against the idea of "everyone finds their soulmate/everyone gets happy" for recurring characters.

I must admit I am too lazy to search for the ages of the other candidates on that list. However, I am sure that Chiba would have recognized Yumis voice when his first love called him, so it is not her.
My guess is on Azusa - there is hardly anything known about her but she is at least appearing more often than the other women mentioned who where only present in one case. And it would be easy for Goshô to show her and Chiba together randomly, similarly to what he is doing with Shiratori and Kobayashi-sensei now.

Nope, it was a manga case, based off of the last file of Volume 61, and the first file of Volume 62.

6th graders are 11-12 (to compare, note how 1st graders like Conan are 6-7).  So he's now 24-25.

Though I can't explain why she'd ask him if he read the letter from thirteen years ago, in that case...

Oh, and I suggest that the girl could be that Kunigami teacher's daughter

IMO Akemi doesn't look that much different, apart from her haircut. Ok, if we don't consider haircuts many characters look similar if not the same, maybe i'm just trying to grasp onto the only chance this can be a plot related case :p
