I think that APTX 4869 only makes the body looks smaller but the cells don't deage. So, they will die when their cells (important ones like cardiac cells) stop working or when a bullet goes into th APTX 4869 takers' head.

Well, I'm pretty sure Haibara said something about the APTX affecting your cells but I guess the bullet thing is right..
