Well... I've met only two people with my name, but since my favorite author's got the same name as I do and there's also a *shudder* politician (Not to offend anyone. I just dislike politicians, because the ones I met are always lying through their teeth ~nya.) with the same name so... 5 ~nya.

Went with 5. My name itself isn't super common (although I did work with 3 people in 3.5 years with the same name), but it's also used as a nickname, so taking that into account, it's about mid-range, I think.
It's at the point where I'm actually shocked when people pronounce it the right way the first time without me telling them. Even though it's not exactly a difficult name to pronounce, following basic English rules. >.>

It really depends for me. Around the world, my name isn't too common. In Israel, there are plenty of people with my name. Then again, Israel is a small country, so I think I'll go with 5.

my name is in the mid 20s on the name popularity lists in the usa and there are a fair number of people in other countries who have it as well, so i'll go with 7. people often call me by a shorter name that is similar (though not quite nickname) and that is also popular.

I've never worked with or been in class with another person who spells their name the same way. No one in my graduating class in high school or college spelled their name the same way. However, it's not like you never see it. You just don't see it too often nowadays.

I'd go with 3. Well, at least over here. Spelling itself doesn't matter, but reverting to the Latin form and allowing different spellings makes it a 6-7, I guess.
