Is this the first time we actually see her adult face. Before when we saw her it always has a shadow over it so we never really saw her face clearly - or am I mistaken?

PS: I'm talking about her actual face - not a photo or anything like that.

You see it well enough in the showdown with Gin on top of the Haido City hotel back in Volume 24. She wears glasses the whole time, but still.

I'm really fond of the Akai = Okiya theory and thus the very last pic of the spoilers confuses me. If I didn't know any other data and was to only see this pic, I'd label Okiya as completely right handed.
Personally I can hardly operate my mouse with my non dominant hand (confusing); same goes for pouring liquid from a bottle into a glass (insecure). Both items are clearly on Okiya's right side. I don't know about the cigarettes, as I have never smoked before, but I think most people prefer to use their dominant hand for that as well. (As long as they don't use both hands simultaneously.) I can drink from a glass with both hands though. 

I understand that he would be using his right hand to brush his teeth when Sera is around. If Akai=Okiya and Sera=his sister theories are true, then it's understandable he would want to deceive her. In this scene though, there is noone around who could be deceived.

It's only been relatively recently that mouses have been able to change to left handed.  Before, you'd have to use your right hand regardless.  It's possible he learned how to do it back then, and just doesn't care about changing from how he learned it.  As for smoking, a lot of people I know (my Dad for instance) use their off hand for smoking, especially if they're naturally right handed.  It allows them to smoke while driving without filling the entire car with smoke (which would be the other way for Japan, where the off-hand for a left-handed person would make this easier), and by using their off hand they can use their primary hand for other purposes.  And like you said, it's easy to drink with either hand.  I personally usually use my off-hand for the very reason I said above about smoking, so I can use my primary hand on the mouse or whatever.

I figured everyone would notice it was an edit because this wasn't in the full spoilers, it has super crappy pixelated edges, and it breaks a panel border in a distinctly non-Gosho fashion. OTL I was only trying to make a point about the conveniently cut out hat and how it looked similar to Scar Akai's.

...on the other hand, this has distinct possibilities for evil use...  >:D

I noticed this and it annoyed me too. He is drinking left handed, but the positioning of the mouse is off. Maybe the computers were setup by a right handed person like Shinichi or his family before and Okiya is just using what is there. He placed things where he did because he doesn't have enough room on the left hand side?
