Dr Adams is actually Simon and decides to die in the fire and allow conan to escape. His message later got me in tears when it said at the end: "Thanks little detective"
Next was the episode where the man blamed his dog Jack for the death of a college student and was almost put down.
the locked room murder case, the karaoke bar murder case and the AO about a duo where the girl killed her partner not knowing that her partner is going to marry her..
Off the top of my head, probably the case where the bride committed suicide due to her groom being her twin. There are sadder ones but I watched this recently so this came up on my mind.
This one woman who Ran met at the market often was killed by a man out of jealousy. He liked her and wanted to be in a relation, but she said she was already in one and actually having dinner with him that night. Later that night when there were firetrucks driving by, the man saw that the woman wasn't with anyone - she was alone on her balcony.
She actually was having dinner with her loved one, but only in spirit. He had died in an avalanche a year (or a few years) ago, and they had promised to have dinner on the 25th after he came back. What made it saddest was that Ran was the one who basically deduced the case and explained it. Especially sad was when you saw the dead lover leaving on his trip, smiling and waving happily, with Ran's words "There was an avalanche. They didn't find his body" right over it ;_;

Spider Mansion. Easily. I mean...I almost think there's one I'm forgetting that was sadder but I can't think of it. It's not even necessarily the tragedy of that case by itself. It's revealed unknowingly from little kids instead of actually showing the scene where he's told why the girl killed herself and then you just go to the culprit repeating to himself "if only I was Japanese, if only she was America".
There's a lot of similar but sad episodes and a lot of similar but cliche episodes. Cant think of specifics but I thought that the pilot episode actually had a murderer on the roller coaster had a lame motive, and even thought the girl was crying i just thought...........wow. But, like the spider mansion lots of murders were sad. Especially Akemi's death

Metropolitan Police Detective Love Story 4 (The one where Satou's having an arranged marriage meeting with Shiratori, and Takagi has to get there before sundown or else she has to marry him.)
Man I have to rewatch the older episodes again.
EDIT: And The Desperate Revival
How could I forget that one?
