I thought the case where Akemi died wasn't too sad at the time it came out, especially since at that time a person basically died every case, but then, with the way that several of the characters like Ai and Akai coped with her death, her death became more meaningful.

I try not to get depressed over an anime till it's either done or its cancelled.... Then i weep and get depressed and watch another entire anime to make up for the loss, only to fall again after i finish watching that anime.....

Saddest case for me would be either the Akemi died, as Conan knew what was happening, but was unable to prevent it. Moonlight Sonata was pretty sad too, though I would that case more under "creepy" than "sad".
