It was only ever on adult swim. The same broadcasting block that kept the original names of characters in other shows such as Bleach, Blood+, Code Geass, Death Note, FLCL, Inuyasha, Lupin the Third, YuYu Hakusho, as well as others which likely kept their character names but I haven't seen to confirm such as Blue Gender, Eureka Seven, Evangelion, etc.
It doesn't sell well enough to warrant redubbing for unreleased sets where they actually could change names, as well as fix errors that occur because of japanese/english angles, so why would they redub just for names? From volume 19 (I think that's the one where Kazuha is introduced) and onwards in the manga, names stopped being changed unless they already had been. Despite this, Funimation still changed her name and everyone else's name in that episode.
Funimation bought the rights to the series and the people they purchased it from got to call the shots on what changes were going to be made when it was dubbed as part of the licensing deal, then Adult Swim bought the modified episodes and aired them.
Adult Swim just bought a lot of other Anime that the licensees had the rights to dub without name changes.
