The move white should have done here if he was not going to exchange bishops is B-Q3 (D3) and if the Knight at f6 goes to E4 then BxN (d3xe4). I agree a Queen side castle is in order to move the Rook to Q1 (d1) but first you need a better hold on e4 and while your queen is watching it you are not going to use it to remove the knight when it comes to E4. This move B-d3 also has threats of the a fore mentioned king side attack. If Black does not move the Knight to E4 after the Bishop move then the Queen Side Castle should be the next move as it sets up a Pin of the queen pawn, and removes the guard of E4 with this Pin (when the Rook is at D1 and the Black Queen is at D8, any capture by the Queen Pawn (D5) allows RxQ (d1xd8).
