actually, she still appeared that way in the anime, back in episode 13. However, she isn't killed by Gin during that episode in the anime (because gin isn't in the episode), so she's reintroduced in 128 to be properly killed by the BO.

she uses the name Hirota Masami, however at the end of the episode when she encounters the mastermind of the operation, he calls her by the name Miyano Akemi. He only says it once though so it's easy to miss.
Really, this confusion would not have happened if they just made the anime follow the manga.
In the manga, Episode 128 didn't happen. The premise of Episode 13 ends with Akemi (who was disguised as the little girl looking for her father) being shot by Gin and dying. No one gets arrested.

Basically take the last few minutes of 128 and have that replace the last few minutes of 13 and that was the manga version.
